Official form to be completed by the scientific committee formed to study the application file of a faculty member applying for academic promotion. It includes an evaluation of scientific output and academic performance according to the criteria of the approved regulations. The form includes the member's data, the rank applied for, the results of the review of research and publications in terms of originality, quality, and fulfillment of the minimum required, and an evaluation of teaching performance and academic participation. At the end of the form, the committee issues a clear and reasoned recommendation, either for approval or rejection, indicating the reasons, and it is signed by all committee members. This form is considered a mandatory document without which the promotion file is incomplete. It is submitted along with the recommendation of the Permanent Promotions Committee to the Scientific Council for approval. It targets heads and members of scientific committees formed for promotions, and the secretariat of the Permanent Promotions Committee.
