The form includes a summary of accomplishments during the period, supporting documents such as copies of research papers, attendance certificates, or scientific reports, and a declaration by the member confirming the accuracy of the data. Accreditation of this report is a prerequisite for fulfilling the requirements of the sabbatical or research leave and for its inclusion in the academic file.
