Specialized evaluation form to be completed when considering requests for service extensions for contracted faculty members who have reached the age of sixty or have completed ten years of service. This form assesses the continued academic effectiveness and institutional relevance of these faculty members. The form includes indicators of academic effectiveness in teaching, research, and supervision, an assessment of academic impact and scholarly output during their years of service, and the department's and college's opinion on the feasibility of the extension. This form provides the Permanent Extension Committee with a documented database to enable them to make a decision based on objective facts. It is intended for college administrations, department heads, and the Permanent Extension Committee of the Scientific Council.
