Official evaluation form to be completed by the department head and college dean to assess the teaching, academic, and professional performance of a faculty member applying for promotion during the measured time period. Evaluation criteria include: teaching quality, faculty member's participation in academic activities, curriculum development, student supervision, service to the department, college, and community, as well as professional conduct and responsiveness to administrative requirements. Evaluations are submitted on a quantitative or qualitative rating scale that allows for the comparison of performance against required standards. The form is a mandatory document within the promotion file and is submitted with the recommendation of the Permanent Promotions Committee. It is intended for department heads, college deans, and the Permanent Promotions Committee of the Scientific Council.
