This report provides a comprehensive narrative of their academic, research, supervisory, and community service achievements throughout their university career, with an emphasis on recent years. This form allows the member to present their case independently to the extension committee with documented evidence, ensuring transparency and fairness in the extension decision. It is attached to the evaluation form and extension request in a single integrated file submitted to the Permanent Extension Committee. This targets Saudi faculty members applying for service extensions.
